Support Image Attachments in Questionnaire Items for Identification Questions

Problem Statement Questionnaires currently support only text-based questions. Educators and administrators cannot create visual identification questions (e.g., identifying pointed parts on diagrams, labeling objects, or inspecting diagrams/photos), restricting assessment formats to text-heavy prompts.

Proposed Solution Enable image uploads within the questionnaire editor for individual question blocks. Authors can insert visual diagrams or photographs with markers (like arrows) to test visual recognition and identification skills.

Acceptance Criteria

  • Media Upload: Add an "Attach Image" button to the question builder supporting standard formats (JPEG, PNG, WebP) with file size limits (e.g., max 5MB).

  • Question Display: Render attached images clearly inline above or beside the question prompt on both digital previews and print/export formats.

  • Layout Scaling: Automatically scale and maintain image aspect ratios without distortion or edge clipping.

  • Answer Sheet Integration: Keep images confined to the questionnaire layout while preserving standard question/answer numbering on corresponding answer sheets.
